Health Insurance If you would like us to file your employer issued health insurance claim:
Click here and we will e-mail the form to fill out with the necessary information needed to file your claim. We charge your credit card upon shipment and then bill your insurance company for you. We will contact your doctor to obtain a prescription. Reimbursement checks will be sent directly to you from your insurance company.
